ELAGABALUS, (A.D. 218-222), silver denarius, Rome mint, issued A.D. 221, (3.42 g), obv. laureate draped bust of Elagabalus to right wearing horn on head, around IMP ANTONINVS PIVS AVG, rev. around P M TR P IIII COS IIII P P, Elagabalus sacrificing to left over altar with a patera, holding branch in left hand, star in left field, (S.7536, RIC 46, RSC 196, BMC 256). Mint bloom on obverse extremely fine, weak on reverse legend, extremely rare.

The reverse could be the coin that Cohen refers to as COS IIII on the reverse as noted in Seaby Roman Coins as in error. This coin clearly has a COS IIII reverse as part of the ethnic.
