Together with St John Ambulance Association Re-Examination Medallion (Fourth Issue type) with five suspended labels for the years 1943 to 1947, reverse of medallion inscribed, 'A111073 Arthur Smithers' and all but the last label inscribed, 'A111073'.

Arthur Charles Smithers, bootmaker, age 21, born at Albert Park, Melbourne, Vic; Enl.31Mar1915 at Melbourne, Vic while currently serving with 26 Aust Sig Engineers; Emb.17Jun1915 with Amb Clearing Hosp; to 1 Aust Gen Hosp Amb Details 17Jun1915; TOS 13 F.Amb 27May1917; various stints in hospital for sprained ankle (4/5/16), burns to arm (18/6/16), and Lobar Pneumonia (15/9/18); RTA 11Dec1918 on hospital transport 'Saxon'; Disch.30Mar1919 Medically Unfit.
